Authorization and Compliance Agreement

By completing this EHS Analytics registration form, I hereby confirm that the information provided, including the organization name and WCB firm number, has been explicitly authorized by the trustee to grant access to its Workers' Compensation Board (WCB) data, training records, and safety information through EHS Analytics.

For the purposes of this registration, the trustee is defined as either the company owner or a senior executive with authority over the organization. The user, as referenced in this form, is an individual authorized by the trustee to access the organization's data. Furthermore, I confirm that I am either the trustee or an individual who has been explicitly approved by the trustee to complete this registration on their behalf.

I acknowledge and agree that this access is granted solely for purposes authorized by the organization and is subject to the following obligations:

• Safeguarding Data: I will protect the organization's data from unauthorized access, misuse, or disclosure and ensure its secure handling at all times.
• Compliance with Privacy Laws: I will fully adhere to all applicable privacy and security regulations, including Saskatchewan's Local Authority Freedom of Information and Protection of Privacy Act (LA FOIP).
